Reasons for Breast Augmentation
There are many reasons you may choose to have breast augmentation. It may be a way of enhancing your appearance, improving the way your clothes fit, or restoring a more youthful shape.
Some specific reasons include:
- Your breasts are asymmetrical in terms of size or shape
- Your breasts are smaller than you would like them to be
- Your breasts have lost volume after nursing or weight loss
- You’d like reconstruction of one or both breasts after a mastectomy

Types of Breast Augmentation
There are two ways to increase the size of the breasts:
- Implants (saline, silicone, structured)
- Fat transfer
Each of these methods has some specific advantages and is appropriate in different situations. For example, the type and placement of implants can affect the shape of the breasts and how they feel to the touch.
Saline breast implants are “pockets” made of thin, flexible silicone. They are typically inserted into the breasts while empty and are then filled with saline (salt water) once they’re in the right position.
The primary advantage of a saline implant is that if the implant ruptures, the body simply absorbs the salt water, and the patient then pees it out. The other advantage to saline implants is that the incisions to place the implants are often smaller than the incisions for silicone implants because the implants are initially put in deflated and then inflated inside the body. This results in a slightly shorter scar.

Silicone implants differ from saline implants in many respects. They are filled with silicone gel, a very thick gel designed to resemble the feel of human fat. Rather than being filled once inserted into the breasts, silicone implants are pre-filled and are inserted as ready-made units.
Silicone implants tend to cost slightly more than saline implants. Many women prefer silicone breast implants because they look and feel more natural than saline implants. While there were concerns in the 1980s about liquid-filled silicone implants, the thicker, cohesive gel silicone implants that are used today have been shown in numerous studies to have no significant health risks. Not only are they safe, but silicone implants today have been shown to have lower rupture rates than the saline breast implants on the market!
MENTOR manufactures both MemoryGel® and MemoryShape® silicone implants. MemoryGel are round implants available in smooth and textured varieties. MemoryShape are teardrop-shaped implants that come in five different height and profile combinations.
Gummy Bear Silicone Implants
A gummy bear silicone implant is a special kind of implant made from high-strength silicone gel. It’s named after gummy bear candies because the consistency of the silicone resembles the consistency of the candy.
The silicone gel that gummy bear implants are made from is thicker and stronger than that used in earlier versions of silicone implants. This means that the implants hold their shape better than other silicone implants and can create breasts that are both fuller and firmer. These implants are also much less likely to break and leak and are therefore safer than other kinds of silicone implants.
Gummy bear implants also compare favorably to saline implants. They typically feel softer and more natural and are less likely than saline implants to cause the development of visible wrinkles or folds in the skin of the breasts.
Sientra®’s gummy bear implants are filled with high-strength cohesive (HSC) silicone gel, which holds its shape while looking and feeling soft and natural, whether they implants are round or teardrop-shaped.
The thing that makes structured implants unique is in their name. While they are filled with the same saline as saline implants, they have multiple shells, rather than just one, making them structured. Saline is inserted between each shell. This structure results in an implant that feels and moves more naturally.
Another benefit to structured implants is their added safety. Because of the extra shells within each implant, their chances of deflating or rupturing decrease. While those are relatively rare problems with any type of implant, some women prefer to be cautious.
The IDEAL IMPLANT® is a structured breast implant available in sizes up to 675 cc and built with at least 3 shells.
Breast Augmentation with Fat Transfer
This procedure doesn’t involve the use of implants at all; rather, it uses the transfer of fat from other parts of your body to your breasts to augment their size and in some cases improve their shape.
During the procedure, liposuction is used to harvest fat from other places on your body, such as your thighs, stomach, or buttocks. This fat is then injected into the breasts using a fine needle.
One of the advantages of this method of this procedure is that it allows the breasts to be sculpted and shaped in a way that the use of implants does not allow. There is also very little scarring, as no large incisions are made.
Your Pre-Surgery Consultation
Deciding between implant type and fat transfer isn’t the only choice you’ll make. You’ll also need to think about the size and shape of the breasts you want.
As part of your consultation, Dr. Obaid will discuss with you the different types of implants and walk you through the decision-making process in order to ensure that the decision you make is your own, fully-informed choice. Seeing Your Results in 3D and Virtual Reality
Making the right choice can seem overwhelming if you don’t know what you’ll look like after your procedure. Thankfully, our office gives you access to this essential information. With the Crisalix 3D simulation virtual reality system, we offer you the chance to see how your new breasts will look on your body before you undergo the procedure.
With just three photos taken during your consultation, Dr. Obaid can create a three-dimensional image of your body, so you can see how the various implants and shapes will look on your form. This allows you to make an informed decision about the type of implants you want and the size and shape you want your new breasts to be.
You’ll have some other decisions to make too, one of which is deciding where your scar will be placed. Typically, you will have four different options for scar placement: belly button, armpit, areola, or the fold underneath the breasts.
Another important decision is whether to have your implants placed over or underneath your pectoral muscles. Dr. Obaid favors under-muscle placement, which tends to provide a softer, more natural look and is associated with fewer complications. However, some people may benefit more from over-muscle placement, specifically if they are involved in heavy-lifting recreational activity or exercise or if they have had previous over-the-muscle implants.
Preparing For and Undergoing Your Breast Augmentation Procedure
Prior to your procedure, you’ll attend a consultation session with Dr. Obaid in which you will select the type of breast implant, the size, the approach used to place the implant, the location (over or under the muscle), and decide if a breast lift is necessary with your breast augmentation.
If you smoke, you’ll be asked to stop for the month prior to your surgery and for six weeks after the surgery. This is important because smoking reduces blood oxygenation, which can slow down healing. If you take blood thinners, you’ll likely be required to stop taking them for the two weeks prior to the surgery. You will be given all of your prescriptions prior to your surgery, and one of our nurses will discuss the effects of all of the medicines and their potential interactions with any medications that you are taking from other physicians.
On the day of your procedure, you’ll meet with your surgeon again to review the process and discuss any questions you might have.
Breast augmentations are performed under general anesthesia, which means you will be totally asleep and will not feel a thing during the surgery. Once it’s complete, your breasts are bandaged and then covered with a surgical support bra. Finally, you’ll be moved to a recovery room while you awaken from the anesthesia.
After Your Procedure
The surgery is an outpatient procedure, which means you can go home on the same day, after a short recovery period. You’ll need to have someone drive you home, and you may need help doing certain things for the first 24 hours. Most people experience only minimal discomfort afterward, and while you may need some pain medication — and plenty of rest — for the first day or two, you’ll likely be back to normal the day after the procedure.
One week after your surgery you’ll have a follow-up with Dr. Obaid to ensure that your body is healing properly. This will be your first chance to see your new breasts. You’ll notice some bruising and swelling, which can be difficult for some patients to see, but within a few weeks, your breasts will be healed and looking great.
Safety and Longevity
All surgical procedures can have complications, and breast augmentation is no different. While most women do not experience any serious complications, possible risks include contraction or rupture of the implants, infection, skin wrinkling, tissue damage, and alteration of sensation in the breast area. Some of these complications may require that implants be removed, repositioned, or replaced.
However, it’s important to note that silicone implants in particular are one of the best-studied medical devices. Wide-ranging studies have shown that these implants are safe and effective, with a low risk of complications. There are no known long-term health effects.
Once placed, breast implants can last a long time — even two to three decades — but there’s no way to predict exactly how long implants will last once they’re placed. On average, you can expect your implants to last for around 15 to 20 years. Fat transfer techniques can produce more results that last longer; however, the results of these procedures can alter over time in some circumstances; for example, if the patient gains or loses weight or has a child.
